§ 153.241 CONTENTS OF APPLICATION.
(A) The application for a zoning permit shall be signed by the owner or applicant attesting to the truth and exactness of all information supplied by the applicant.
(B) The following information is the minimum required:
(1) Name, address and phone number of applicant;
(2) Legal description of property;
(3) Existing and proposed use;
(4) Zoning district:
(a) Current zoning; and
(b) Proposed zoning.
(5) Plans showing the following dimensions:
(a) Building site area;
(b) Building site width;
(c) Building site coverage;
(d) Building height; and
(e) Building setback lines.
(6) Number of off-street parking spaces or loading berths;
(7) Number of dwelling units; and
(8) Any other matters which may be necessary to determine conformance with and provide for the information of this chapter.

§ 153.242 APPROVAL.
Within five working days after the receipt of the application, the Zoning Inspector shall either approve or disapprove the application.

§ 153.243 INTEGRATION OF ZONING/BUILDING PERMIT.
The zoning permit will be integrated with the building permit. The zoning/building permit will be issued based on compliance with zoning regulations and building codes.

§ 153.244 RECORD OF ZONING PERMITS.
The Zoning Inspector shall maintain a complete record of all zoning/building permits, and copies shall be furnished or presented for review to any person making a request.

§ 153.245 FAILURE TO OBTAIN PERMIT.
Failure to obtain a zoning/building permit shall be a violation of this chapter and will be punishable under the provisions set forth in § 153.999.

§ 153.246 CONSTRUCTION AND USE TO BE AS PROVIDED IN APPLICATION, PLANS AND PERMITS.
Zoning/building permits issued on the basis of plans and applications approved by the Zoning Inspector authorized only the use of arrangement set forth in the approved plans and applications. Any use, arrangement or construction contrary to that authorized shall be deemed a violation of this chapter and shall be punishable under the provisions set forth in § 153.999.
