DECORATIVE LANDSCAPE CURB (NTS)
NOTES:
1.   All exposed concrete shall have a broom finish.
2.   Contraction/control joints shall be grooved every four feet and sealed with polyurethane sealant limestone color equal to sonolastic.
3.   Fill cannot be placed higher than the top of the decorative landscape curb without design engineers calculations for stability of curb.
4.   Inspection must be scheduled at least 48 hours before pour, with the village, to ensure application of bond coat and proper location.
5.   Decorative landscape curb to be bonded to asphalt surface by use of SS-H or CSS-1-H Cutback asphalt per I.D.O.T. Article 1009.07. Apply tack coat in a manner to provide a uniform continuous coating one-eighth inch to three-sixteenths inch in thickness and a width one inch less than the base width of the curb. Care should be exercises to prevent spills or running of tack coat over surface of finished asphalt.

A-7 Street Sign Specifications

All street identification signs within the Village of Shiloh shall conform to the following standards:
1)   Sign shall be .080 aluminum conforming to 5052 # 38 alloy Engineer grade.
2)   Sign shall be six feet tall with four-inch high lettering conforming to Highway Gothic B.
3)   All Suffixes: Drive, Court, Lane etc. Letters to be two inches high Highway Gothic B and shall follow the National Standard for said abbreviation.
4)   Red background to be Premium Fire Red #2076 as manufactured by Spar-cal or approved equal.
5)   White to be Reflective 3M Engineer Grade #3260 or approved equal.
6)   All signs to be U.V. protected.
7)   Edge border to be three-eighths inch wide and white in color as described in #5 herein.
8)   Letter and logo to be equally spaced from edge of sign with a minimum space of one-half inch from inside edge of white border of letter or logo.
9)   One-half inch space shall also be between logo and 1st letter of street name.
10)   Minimum space of one-half inch between top of lower white border and letters or logo.
11)   Minimum space of one-half inch between bottom of upper white border and letters of logo.
12)   Spacing between letters to conform to national standard.
13)   Letters can not be condensed to less than 90% of specified size.
14)   Sign artwork to conform to Exhibit A.

D-1 Sub-grade

   It is assumed that all sub-grades will be properly graded to the lines and grades shown on the plans for the given project. In the event there is to be embankment construction, all fill material shall be placed and compacted per the requirements of the IDOT Specifications for Road and Bridge Construction or Shiloh Codes whichever requires the highest level of performance.

D-1 Forms

   Forms shall be clean and securely staked and braced prior to placement of concrete. Forms may be wood or metal, straight and free from bend or warp. Deviation from true straightness shall not exceed the following limits for a ten (10) foot length as determined with a stringline.
Horizontally   1/4 in
Vertically   1/8 in
   When set to grade and staked in place, the maximum deviation of the top of any section from a straight line shall not exceed 3/16 in. The forms to be used shall be capable of carrying the weight of the paving equipment without deflecting or being depressed into the sub-grade/sub-base.

D-1 Equipment

   Portland cement concrete pavement may be placed with a slip-form paver with an automatic grade control system. The slip-form paver shall be capable of striking off, consolidating, and finishing the pavement to the required cross section. Pavement placed with this method shall be checked with a ten (10) foot straightedge and any necessary corrections made as discussed hereinafter.
   At the Contractor's option, in lieu of a slip-form paver, he may use forms in conjunction with the following equipment to place Portland cement concrete:
   a.   A two-screed power driven finishing machine designed to strike off, screen, and consolidate the plastic mixture, or
   b.   A vibrating screed to strike off and consolidate the mixture to the proper cross section, and
   c.   A longitudinal float not less than twelve (12) feet in length and six (6) inches in width that is properly stiffened to prevent flexibility and warping, and equipped with handles.
   In addition, with employment of the equipment specified in either (1) or (2) and (3) above, the Contractor shall be required to furnish at least two 10-foot metal straightedges for use following either the finishing machine or the longitudinal float. The straightedges shall not exceed true straightness by more than 1/16 in. when checked with a stringline. Long handled wood or metal float shall be used to correct a surface deviation indicated by the straightedge. Said floats shall have a blade not less than five (5) feet in length and six (6) inches in width.

D-1 Sub-grade Preparation

   Prior to placement of concrete on the sub-grade/sub-base, the grade shall be checked for proper depth with a template or stringline if forms are used, or with before and after elevation shots for slip-form work. Any soft or spongy areas shall be properly corrected by undercutting or stabilization. Under no circumstances shall concrete be placed on a frozen sub-grade. Provisions shall be made for wetting dry sub-grades in advance of concrete placement.

D-1 Placement & Finishing

   Plastic Portland cement concrete shall be deposited on the grade in such a manner as to require a minimal amount of rehandling or segregation. It shall be deposited in successive batches in a continuous operation. No batch/load shall be incorporated that has been in the truck longer than sixty (60) minutes. The operation will be coordinated in such a manner as to have the mixture struck off and consolidated within thirty (30) minutes of deposition on the grade.
   If a vibrating screed is used to strike-off and consolidate the mixture, the Contractor may be required to make two passes with the equipment. The longitudinal float shall be passed across the entire surface a minimum of two times with an overlap of 50% between successive locations along the pavement.
   After the operation of the slip-form paver, or the finishing machine, or the longitudinal float, but while the concrete is still plastic, the pavement surface shall be tested with a ten (10) foot straightedge. The straight-edge shall be held in successive positions parallel to the pavement centerline in contact with the surface. Advancement along the pavement shall be such to provide at least 50% overlap for successive stages. High areas shall be cut down and refinished with a five (5) foot float. The corrected area shall be checked again with the ten (10) foot straight edge.
   All construction joints and pavement edges shall be finished with an edging tool having a radius of 1/4 inch.
Subsequent to initial set, and prior to hardening of the surface, the pavement shall be given a broomed or burlap drag finish either transversely or parallel to the centerline of road. Resulting indenture of the surface shall yield a nominal depth of 1/16 inch.

D-1 Curing

   Subsequent to application of the finished texture, the freshly placed concrete shall be covered with polyethylene sheeting or curing compound as soon as such application can be made without marring the surface. In the case of curing compound, the Contractor shall insure that adequate and uniform coverage is achieved. In the case of polyethylene sheeting, all edges shall be securely anchored to prevent air movement under the sheeting. Curing shall be maintained for a minimum of 72 hours. Polyethylene sheeting may be temporarily rolled back to allow sawing of contraction joints. The exposed surface shall be wet and recovered for the remainder of the curing period subsequent to sawing.
   When freezing temperatures are predicted within 72 hours of placement of concrete pavement, adequate insulation shall be provided for protection against freezing.

D-1 Joint Sealing

   Prior to traffic being allowed on newly placed concrete pavement, all joints (contraction and longitudinal) are to be cleaned and sealed with hot poured rubber asphalt joint filling compound.

D-1 Portland Cement Concrete

   All Portland cement concrete to be used for pavement construction shall be a 6-bag mix meeting the following mix proportions with the aggregate meeting the gradation and quality requirements of the Illinois Department of Transportation and must attain 4000 psi in 28 days.

   In addition to being proportioned in accordance with the above requirements, the plastic mixture shall conform to the following requirements:

   Calcium Chloride shall not be used in concrete mixtures except in cold weather construction and at that time shall not be in excess of 2%.

E-1 Mix Proportions

   Portland cement concrete to be constructed in subdivisions for curb and/or gutter, sidewalk, paved ditch, drainageways, median, etc., shall conform to the following approximate mix proportions per cubic yard and attain minimums of 4000 psi in 28 days.

E-1 Construction Methods

   All Portland cement concrete shall be placed at such a consistency that the plastic mixture is workable but does not exceed the slumps stated above. Concrete shall not be placed that has been batched in excess of one (1) hour. Any truck shall be empty within one and one-half (1-1/2) hours after batching.
   Placement shall be accomplished in an acceptable and workmanlike manner. Care shall be exercised to avoid segregation of the mixture. The mixture shall be properly consolidated (by vibrator or spading) prior to or concurrently with the strike-off operation.
   Forms shall be clean and securely staked and braced prior to placement of concrete. Forms may be wood or metal, straight and free from warp. Deviation from true straightness shall not exceed the following limits for a ten (10) foot length when checked with a stringline:
      Horizontal     3/8 inch
      Vertical     1/4 inch
   Forms shall be stable enough to prevent vertical movement during placement and finishing operations.
   All concrete shall be worked sufficiently to bring 1/8 inch to 1/4 inch of mortar to the surface to allow for proper finishing. Prior to the final set, the finished product shall be given a lightly broomed finish at right angles to the centerline of the road or to the flow line.
   All freshly placed concrete shall be properly cured with polyethylene sheet (properly secured) or a membrane curing compound with a fugitive dye for a minimum period of 72 hours. Curing application shall be accomplished as soon as it can be applied without marring the finish.
   Calcium chloride shall not be used in concrete mixtures except in cold weather construction and at that time shall not be in excess of 2%. Portland cement shall not be placed on spongy or frozen grade. When freezing temperatures are predicted within 72 hours of placement, adequate insulation shall be provided for protection against freezing.
   Curb and curb & gutter shall have two #4 reinforcing bars, twenty-four (24) inches long, placed at all construction joints. Joints in curb and curb & gutter shall coincide with those in adjacent P.C.C. pavement. For curb and curb & gutter joints adjacent to flexible drive or parking surfaces, joint spacing shall not exceed 20'. All joints are to be sealed.

SECTION 1. Appendix F of Chapter 151 - Requirements for Demolition

"For a permit for the demolition of a building or structure, the fee shall be a rate of $50.00" for each building or structure, which includes inspection.

SECTION 2. DEMOLITION

A)   A person obtaining a Permit to Demolish a building from the Village of Shiloh is hereinafter referred to as "Permittee."
B)   A "Permittee" shall obtain a Permit for demolition of a building or structure from the Building Official of the Village of Shiloh. The Permit shall require such information as the Building Official deems necessary. Before a Permit shall be issued, the "Permittee" shall submit proof to the Building Official that all the utilities connected to this structure or premises have been notified of the impending demolition.
C)   The "Permittee" is responsible to first notify all Utility Companies of the demolition prior to beginning the project. All rules and regulations of the Utility Companies along with Village and County Codes must be complied with before, during and after the demolition project.
D)   The "Permittee" shall fill only with broken concrete, masonry rubble, sand, gravel or clean dirt up to 36" from the level grade of the existing surrounding area. Any concrete used for fill shall be no larger than 6" on any side and shall be so placed to minimize voids. Broken concrete or masonry rubble shall not be placed in larger than 12" lifts with a minimum of 12" of compacted gravel, sand or dirt placed over each lift of broken concrete or masonry rubble. Compaction of gravel, sand or dirt to be a minimum of 90% of standard lab density. Contractor to pay for and supply compaction test results from a certified testing laboratory. This shall include basement floors. The top layer of fill shall be a minimum of 12" soluble soil, capable of encouraging vegetation. Site shall be seeded and watered until vegetation is established.
E)   The "Permittee" shall comply with all regulations of the Illinois Environmental Protection Agency and the United States Environmental Protection Agency.
F)   Lumber, lathe, or other organic materials will not be permitted in the fill.
G)   Material which is not suitable for fill material shall not be used, but shall be removed from the site of the work and disposed of at the "Permittee's" expense.
H)   All of any materials to be disposed of shall be disposed of at a State of Illinois approved disposal area.
I)   Any authorized material remaining on the premises, lack of final grading and/or lack of vegetation will delay final acceptance until such problems have been corrected.
J)   The burning of materials is not authorized or permitted.
K)   Care shall be taken by a "Permittee" to prevent the occurrence of voids in the fill or undue settlement after the fill has been placed.
L)   The operation of the "Permittee" shall be done in such a manner as to avoid hazard to person's and properties. Interference with the use of adjacent buildings or the interruption of free passage to and from such buildings is prohibited.
M)   After work is started on the building, or structure, it should be continued promptly and expeditiously. The "Permittee" shall satisfactorily complete the work within thirty (30) days from the date of issuance of the Permit. If said work is not completed, the Village shall make a claim to have the bonding company complete the job.
N)   The "Permittee" shall, at all time, reserve and comply with the Federal and State Laws, Local Ordinances, and Regulations which effect the conduct of his work.
O)   The "Permittee" shall be solely responsible for all claims and liabilities arising from or based upon the violation of any such laws, ordinances, or regulations whether by himself or his employees.
P)   The "Permittee" shall provide the Village with a Performance Bond in the amount to be determined by the Village Staff. Said Bond is to be approved by the Village Staff to guarantee that the "Permittee" shall comply with all the terms of this Ordinance.
Q)   Under no circumstances shall debris, mud, rock, dirt or trash of any kind be on Public Right-of-way. It shall immediately be removed by "Permittee."
R)   The "Permittee" shall call the Village at least 24 hours prior to beginning work and shall call at least 24 hours in advance to schedule the following required inspections.
   (1)   When concrete is broken up.
   (2)   When fill is being placed.
   (3)   Final.

VERIFICATION FROM UTILITIES: "PERMITTEE" IS RESPONSIBLE TO FIRST NOTIFY ALL UTILITY COMPANIES OF THE DEMOLITION PRIOR TO BEGINNING THE PROJECT. THESE "DISCONTINUANCE OF SERVICE LETTERS" MUST BE ON FILE BEFORE THE PERMIT IS ISSUED.
SANITARY SEWERS: IF A SANITARY SEWER IS UNCOVERED DURING THE COURSE OF DEMOLITION, THE OPENING SHOULD BE PLUGGED. THE OFFICE OF THE DEPARTMENT OF PUBLIC WORKS (632-5667) MUST BE CONTACTED BY THE "PERMITTEE" AND THE PLUG INSPECTED.
BOND: A BOND IN THE AMOUNT OF                                                      WILL BE REQUIRED.
LIABILITY: THE "PERMITTEE" SHALL PROCURE AND MAINTAIN PUBLIC LIABILITY AND PROPERTY DAMAGE INSURANCE AND FURNISH VILLAGE COPY OF CERTIFICATE OF INSURANCE INDICATING SAME.
THE "PERMITTEE" SHALL COMPLY WITH ALL PROVISIONS OF THE DEMOLITION ORDINANCE (SEE ATTACHMENT) OF THE CORPORATE AUTHORITY.
THE "PERMITTEE" AGREES TO SATISFACTORILY COMPLETE THE WORK WITHIN THIRTY (30) DAYS FROM THE DATE THE CONTRACT IS AWARDED.
